In this role, you will utilize critical thinking skills to select the correct reason for assessment, Assessment Reference Date (ARD), and reasons for assessment. You'll work in conjunction with the entire care team to ensure that the care/services provided are captured accurately & appropriate payment is received by the Medicare/Medicaid systems. You will disseminate any new or updated materials involving the Resident Assessment process & trains the other interdisciplinary team (IDT) members as needed. On a monthly basis, you will review MDS's for accuracy, analyzes QI/QM data in conjunction with the Director of Nursing Services to identify trends. You'll also ensures the timely electronic submission of all Minimum Data Sets to the state data base, saves & reviews the state validation reports and ensures that appropriate follow-up action is taken.


  • Licensed as an LPN or RN
  • Previous experience in a long term care facility
  • Previous long term care experience preferred
