Guardian Home Health Expectations for Certified Nurse Assistant  

 The Home Health Certified Nurse Assistant is responsible for providing patients with in-home personal care and designated health-related services to maintain the patient's physical and emotional wellbeing while following the written plan of care, Medicare/Medicaid regulations and company policies and procedures. Guardian Home Health is the preferred post-acute care partner for skilled nursing facilities, hospitals, physicians and families nationwide. From home health community-based services, we deliver high-quality, cost-effective care that empowers patients to manage their health at home.

  • Reports any unusual circumstance in the patient's condition or home environment to the primary RN/Patient Care Manager 
  • Follows currently written an aide assignment sheet to provide personal care and assistance with activities of daily living (ADLs) to include mobility transfer, walking, grooming, bathing, dressing or undressing, eating, or toileting  
  • As assigned, performs incidental household services essential to the patient's health care at home that is necessary to prevent or postpone institutionalization  
  • Completes a clinical note for each visit within required timeframes, which must be incorporated into the patient's record 
  • Participates in the daily report. Adheres to and supports the care management model within the company 
  • Participates in a staff conference regarding patient care  
  • Provides care according to the aide plan of care assignment sheet and under State Practice Act or regulations 
  • Records and notifies Inpatient Care Manager of any variations from the established parameters or changes in the patient's condition  
  • Provides care according to the aide plan of care assignment sheet and following State Practice Act or regulations 
  • Records and notifies Inpatient Care Manager of any variations from the established parameters or changes in the patient's condition 
  • Current driver's license, vehicle insurance, and access to a dependable vehicle 
  • Must be mature and able to deal effectively with the demands of a Home Health Certified Nurse Assistant position. Able to read, write, and carry out directions promptly and accurately  


Minimum of one (1) year of experience as a Certified Nurse Assistant is required for this position
